Referral information
Refer with consent, clarity and care.
This pathway helps individuals and organisations prepare an initial referral enquiry while protecting the person's dignity, agency and privacy.

Referral readiness
Four checks before making contact.
These checks help keep the first conversation appropriate and focused on the safest next step.
- 01
Confirm knowledge and consent
Wherever possible, the person should understand the proposed referral and agree to the initial contact.
- 02
Use proportionate information
Share only what is necessary to understand the broad purpose, suitability and access considerations.
- 03
Explain the intended outcome
Describe what the person hopes will change without diagnosing them or writing a detailed case history.
- 04
Separate urgent risk
Use appropriate emergency or safeguarding routes first when someone may be in immediate danger.
Appropriate initial context
Begin with the minimum information needed.
- Your name, role, organisation and safe contact details
- Your relationship to the person being considered for referral
- The broad support area and the intended outcome
- Relevant access or communication considerations
Do not send initially
Detailed or unnecessary personal records.
A general web form is not the place for clinical notes, identity documents, detailed safeguarding disclosures or extensive case histories. If further information is appropriate, Thrivewell can explain a suitable route after the initial review.

What happens after an initial enquiry.
- 01
Suitability review
We consider the broad request, intended outcome and whether Thrivewell may be an appropriate route.
- 02
Proportionate clarification
Where needed, we ask focused questions and explain how any further information should be shared.
- 03
Next-step guidance
We explain an appropriate next action or, where Thrivewell is not the right fit, another direction to consider.
Initial referral enquiry
Establish the right route before sharing a case history.
This form is for preliminary suitability and process questions. It is not a clinical referral record or an emergency reporting channel.
Immediate risk needs an urgent local response.
Thrivewell is not an emergency service. Contact the appropriate local emergency or safeguarding authority first if a child or adult may be at immediate risk of harm.
